Anyone tell me why I'm getting this?

I'm not doing anything differently than I normally do but when I extrude an edge I get extra edges and geo as well.. I smoothed the project I'm working on and their were extra edges where their aren't supposed to be and when I grab a face and pull it out their is geo behind it attached to it as well. I reset to default and it still does it..

I opened a new project and extruded an edge on a plane and it gave me eight extrusions, (and I never set it to eight, even in a previous project) so I reset it again and it seems to be working but I'm going to have to keep checking every extrusion and start all over again because the previous project, the extrusion won't reset itself, even if I manually do it in the extrusion window and the pref. window..

Make sure "Keep Faces Together" is checked under the Edit Mesh menu.
