The Soft Modification Tool has appeared in my tool box, and seems to be interfering with the Extrude Face option. The manipulators don't appear when I activate Extrude Face. How do I remove the SMT from the tool box?

Not easy, youll need to edit your GUI. with the tone of your question it sounds like this just started happening. I doubt it is the root of your problem. Post more so we might try and help you.

Yes, this just started happening. The only thing that I have done most recently, is use two CD's from books that I purchased. One is Maya Ultimate Workshop (Maya5), and a workshop from Maya 4.5, My version is 6.5. If I attempt to extrude a face, everything works properly, except no manipulator shows up at the face. I can choose any tool to Move, Rotate, etc. and they will work. But, there is no tool there when I click Extrude Face.

ok dont know what happend but heres a process that should reset your settings.

Go to My documents folder...Maya folder...6.5 folder and change and rename the prefs folder to 'prefs old'. restart maya and you should have a default set of prefs. If this doesnt work then just delete the new prefs folder that was created and name the old folder back to prefs.

thanks Tick, renaming prefs worked by resetting everything, however, Soft Modification remains in toolbox. that does not cause a problem anymore when extruding faces, and that I am pleased about. thanks again.
