Changes to Poly cage not affecting subdiv object

I did a search and could not find a solution to my problem so I hope that someone can help me out.
Here are the steps I am taking and the problem that is presenting itself.
1. I create a subdiv primative cylinder.
2. I activate polygon mode, select faces, select the cap of the cylinder and delete it making a hole or hollow cylinder.
3. I pick either one, some or all of the edges surrounding the hole and extrude them.

Here's the problem. Sometime the extrude effects the subdiv model and sometimes it doesn't. Sometimes the subdivided object will behave just like it is supposed and reflect the changes made to the polygon cage. But other times the polygon cage will change but it won't effect the subdivided surface at all. As you can see in the pic the edge at the top of the breast extended properly but the polygon wireframe shows that all of the other edges of the subdiv object did not.

Things seem to work if I create a polygon primative, extrude the edges and then convert it to a subdivided object but it is much harder to model without seeing how the polygonal object will look when subdivided. Can anybody tell me what I am doing wrong?
