I'm having the same problem, but my history is turned on. I was using the extrude face operation many times, then needed to make a new part, opened a new scene, created a poly cube, Highlighted some faces, set divisions to 4, clicked extrude, no manipulator. I never shut maya down, just opened a new scene. I've been researching the forum for causes or fixes. I have tried renaming the prefs folder forcing Maya to reset all prefs, but that didnt help. Anyone have any ideas what I can look at to make sure I havent set something wrong? Any common newbie mistakes that would cause this? user added image

Try reinstalling your graphics card driver, that might help as sometimes people have dissapearing manipulators and its down to the graphics card.

Hope it helps

Thanks for the replies. My cards are up to date, and I tried the first answer, I turned the history on and off, and closed and opened Maya, and its working. I think when I opened the new scene to make the new part, maybe the history was off, even though the history was on in the original scene. Not sure, but thanks.. its back...(whew)
