From Concept art to 3D

OK, I really need help with something. It has been bothering me for hours now and i can't find a solution. What i want to know is how they make the terrain for the games. I mean if someone gives you a concept art of an environment in perspective view like this one, how do you model the mountains??? how do you make the terrains. This is just for information, im not actually trying to do this. I just want to know cause its been bugging me user added image .

After searching for a long time I found out that you can make heigt maps in photoshop which can then be used to make the environments in unity by importing the height map and use it on your terrain. Now i want to know if this is the way they do it in games.
