Keep faces together dont work.

With checked option it is still extruding separate faces...

But I tried in new scene, and it work correctly.

Is there something I am missing??

It's possible you've already extruded with faces off, scaled, and then when you hit undo, you didn't undo the actual extrude, so there's duplicate faces exactly on top of the original ones, so when you extrude again, they won't be together. Either do a merge, delete the dup faces, or start that piece of geo over.
